Community Impact & Outcomes
Transparency and accountability are core values of Reclaim Hope Foundation. We measure, track, and publicly report the outcomes of every program to ensure our work delivers genuine, lasting impact for the individuals, families, and communities we serve. This report reflects cumulative data from our founding in 2014 through the current fiscal year.
Measuring What Matters
Results by Program Area
Each program is evaluated using standardized outcome measures, participant self-report surveys administered at intake and at thirty, sixty, ninety, one hundred eighty, and three hundred sixty-five day intervals, and independent verification where applicable. All figures below represent verified outcomes reported to our funders and the Colorado Behavioral Health Administration.
Peer Support & Navigation
Since inception, our peer support program has conducted over twenty-six thousand individual navigation sessions and facilitated more than four thousand group circles. Participants who engage for ninety days or more show a sixty-two percent reduction in emergency department visits related to substance use. The program has trained and deployed one hundred twelve Certified Peer Support Specialists into the Denver metro behavioral health workforce.
Workforce Reintegration
Over nine hundred individuals have completed the twelve-week workforce reintegration track since 2016. The average starting wage of eighteen dollars and forty cents per hour significantly exceeds Colorado’s minimum wage. Eighty-one percent of employed graduates retain their position at the six-month mark. Twenty-three participants have advanced into supervisory roles, and fourteen have started their own small businesses with support from our financial literacy curriculum.
Family Reunification
Family reunification outcomes are measured using the Family Assessment Form (FAF) and the Relationship Satisfaction Scale administered at intake, session four, and post-completion. Seventy-six percent of families completing the full eight-session track report sustained relationship improvement at the one-year follow-up. Our children and youth program has served over six hundred young people, with ninety-one percent of participants demonstrating improved coping and emotional regulation scores.
Harm Reduction & Naloxone
Launched in 2018 with funding from the Colorado Department of Public Health and Environment, our harm reduction program operates mobile outreach in twenty-two locations across Denver and Adams Counties. We have trained over three thousand community members in overdose recognition and naloxone administration. Thirty-eight percent of individuals who engage with our harm reduction team subsequently connect with treatment or other recovery services within six months of initial contact.
